- PET - perfusion and metabolism
- SPECT or Tc-99m - preserved cellular integrity
- Thallium-201 - perfusion / viability / cellular integrity
"gold standard" for viability assessment among single photon agents. Poor image quality.
- Dobutamine ekg - contractile reserve
- MRI - contractile reserve, degree of myocardial fibrosis
- Contrast echo - microvascular integrity
- Sestamibi - minimal redistribution
- For acute chest pain imaging, sestamibi or tetrofosmin are required because of their stable myocardial retention, which permits imaging up to three hours after the injection.
- For the detection of viable myocardium, thallium is currently the single photon agent of choice, but sestamibi and tetrofosmin appear to provide useful information as well.
